Route 21 serves the communities of St Jacobs and Elmira. It also connects to [[Grand River Transit Conestoga Station|Conestoga Station]] and St Jacobs Farmers Market.
 
From Conestoga Station, buses operate via north on Kraus Drive, west on Northfield Drive, north and west on King Street, south and west on Farmers Market Road, north on Weber Street, north on King Street, east on Sawmill Road, and north on Arthur Street/Highway 85, looping via west on Church Street, south on Snyder Avenue, and east on First Street to Arthur Street, then following the reverse routing to Conestoga Station.
